The mission of the Upper Peninsula Association of Rural Health Services is to provide high quality primary health care with respect and compassion to all in need.

Our Vision:

Our vision for the UP Association of Rural Health Services is to be a major health provider in the Upper Peninsula of Michigan which offers a coordinated and expanded system of primary care including medical, oral health, nutrition, vision, preventive care, mental health and pharmacy.

Beverly Johnson, DDS

BEVERLY JOHNSON, D.D.S.

Dr. Beverly Johnson attended University of California where she received her D.D.S. degree in 1979. For the next four years she served as a general dentist in various private dental practices in Los Angeles.

Along with her own private practice in Inglewood, CA she also served part-time with various private dental practices in Los Angeles from 1983-2007. For two years, 1985-95, Dr. Johnson served as Expert Examiner for the California State Board of Dental Examiners, Sacramento, CA.

Presently, Dr. Johnson is a “locum tenens” dentist at UPARHS practicing general dentistry. She works in both the Spalding and Menominee clinics. She is the Clinical Director/Dental Director for the Upper Peninsula Association of Rural Health Services, Marquette, Michigan

Barbara Womack, D.D.S.

BARBARA WOMACK, D.D.S.
Dr. Barbara Womack attended the University of Michigan School of Dentistry in Ann Arbor, MI where she received her D.D.S. degree in 1980. For seven years she worked as associate dentist in the Cuthkell Womack Dental Centers in Detroit, MI. From 1988-2002 Dr. Womack was CEO and general dentist for the Cuthkell-Womack Dental Centers.

After selling her practice in 2003 she became a stay-at-home mother until 2007 when she joined the Dental Care Alliance in Detroit. She is presently working as a "locum tenens" dentist wherever she is needed in all of the Dental Clinics of UPARHS.

Laurence Nagy, D.D.S.

LAURENCE W. NAGY, D.D.S
Dr. Laurence Nagy attended the University of Detroit Mercy where he received his D.D.S. degree in 1968 and when he then began his private practice of dentistry.

His first practice was in Plymouth, Michigan from 1968 until 1975, and then relocated his practice to Canton, Michigan. On July 17, 2001 he sold his practice and retired. Since then he has been working on a part time basis.

Dr. Nagy presently works for UPARHS as a "locum tenens" dentist several days each month. He works regularly at the Sawyer Dental Clinic in Gwinn, MI.

Alton Kendrick, D.D.S.

ALTON KENDRICK, D.D.S.
Dr. Alton Kendrick attended the University of Michigan, School of Dentistry in Ann Arbor, MI where he received his D.D.S. degree in 1980. For the next two years he practiced general dentistry for Midwestern Dental of Dearborn. Subsequently he became the Director of Midwestern Dental in Dearborn, Warren, Detroit and returning to Dearborn for his final twelve years as Director.

Since 2002 Dr. Kendrick has been in Private Practice at the Kendrick Family Dental Center, P.C. in Dearborn. He also serves as a "locum tenens" dentist for UPARHS several days each month. Most often Dr. Kendrick works at the Sawyer Dental Clinic in Gwinn, MI.

Stephen Forsyth, D.M.D.

STEPHEN P. FORSYTH, D.M.D.
Dr. Stephen Forsyth attended Southern Illinois University School of Dentistry where he received his D.M.D. degree in 1981. He then began his practice in Charleston, Illinois with the Griffiths-Forsyth Dental Group. He spent twenty years there until he moved to Harbor Springs, Michigan where he was employed as a dentist for Dental Clinics North for the next 3 years.

Since October of 2004 to the present time Dr. Forsyth and his wife have their own dental practice in Gaylord, Michigan called Forsyth Family Dentistry PC.

He is also willing to work for UPARHS as a "locum tenens" dentist several days a month in the dental clinics where he is needed.

Angela Hall, D.D.S.

ANGELA HALL, D.D.S.

Angela Hall received her Doctor of Dental Surgery degree from the University of Michigan School of Dentistry in 2005. Upon graduation from Iron Mountain High School in 1997 Dr. Hall went on to receive a bachelor's of science degree from Central Michigan University with a major in Biology and a minor in Chemistry. Dr. Hall is committed to providing comprehensive care to all of her patients, including everything from examinations to endodontic treatment as well as cosmetic dentistry.

Dr. Hall is a native of Iron Mountain, Michigan and is thrilled to be back in the Upper Peninsula. Dr. Hall and her husband, Dr. Jason Hall, who is an orthodontist in Iron Mountain, Michigan look forward to enjoying snowy winters and warm summers in the U.P. as they begin establishing themselves in the community.

Dr. Hall is presently working for Upper Peninsula Association of Rural Health Services and practices in many of our dental clinics.

CHRISTOPHER C. MAKI

Dr. Christopher Maki attended the University of Michigan, School of Dentistry in Ann Arbor, MI where he received his D.D.S. degree in 1988. For the next year he was an associate with Dr. Stan Gresham, Ironwood, MI and then from 1990 to 1993 he was Dentist, Consultant and Director of Mobile Dental Project for the Western Upper Peninsula District Health Department.

From 1993 to 1996 he was Dentist/Director of Jackson Area Dental Clinic. Since 2000 Dr. Maki is in private practice in New Lothrop, MI. He also serves as a "locum tenens" dentist for UPARHS several days each month. Most often Dr. Maki works at West Mackinac Dental Clinic in Engadine, MI.

 

REGISTERED DENTAL HYGIENISTS

ELLEN MOILANEN, RDH

Ellen attended the Mott Community College receiving her degree as a Registered Dental Hygienist in 1982. Prior to coming to work for the Association in August of 1995, she worked in various dental offices as a Dental Hygienist. Ellen has been a Registered Dental Hygienist for 26 years.

Chantelle Eichhorn, RDH

CHANTELLE EICHHORN, RDH

Chantelle attended Ferris State University, Big Rapids, MI receiving her degree as a Registered Dental Hygienist in 2002. Prior to coming to work for the Association in June of 2005, she worked Knudsen Dentistry in Escanaba and Lakeview Dental Associates in Lakeview, MI. Chantelle has been a Registered Dental Hygienist for 6 years.

Kimberly Browning, RDH

Kimberly attended Baker College of Port Huron, MI receiving her degree as a Regitered Dental Hygienist.

Heather Lynn Seymour, RDH

Heather attended Northeast Wisconsin Technical College in Green Bay, WI receiving her degree as a Registered Dental Hygienist in 2005. Prior to coming to work for the Association in 2006, she worked in other dental offices as a Dental Hygienist.
