Sight For Students Program

Free Eye Exams and Glasses for Children
Many children are unable to get the eye care that they need. The National Association of Community Health Centers has developed a program in conjunction with the Vision Service Plan to address this need.

Who is Eligible

  • Family income is no more than 200% of federal poverty level.
  • Child is NOT enrolled in Medicaid or any other vision insurance program.
  • Child or parent is a U.S. citizen or resident alien with a Social Security number.
  • Child has not used the VSP program during the last 12 months.

What You May Receive

  • Free Eye Exam
  • Free Glasses

How to Obtain Services

If you see that your child has vision problems or this has been found through a screening done through the school, health department or other health care provider, then:

Step 1.

Application – Obtain and complete a short application from the local Medical Center:

  • Ewen Medical-Dental Center – Ewen, MI 906-988-2210
  • Northern Menominee Health Center – Spalding, MI 906-497-5263
  • Northern Menominee Health Center – Menominee, MI 906-863-2315
  • Dickinson-Iron Dental Center – Crystal Falls, MI 906-875-6133

Step 2.

Eligibility Determination

  • If you are enrolled in the Sliding Fee program at the health center for this year, this information will be used to determine eligibility.
  • If you are not enrolled in the Sliding Fee program, you will be asked to provide income information such as tax returns, wage stubs, etc.

Step 3.

Voucher

  • If eligible, you will receive a voucher for your child to use at a participating eye specialist.
